As a Care Management Coordinator, Utilization Review, you will play a critical role in ensuring that our patients receive the highest quality care while minimizing unnecessary hospital stays. You will be responsible for coordinating, communicating, and facilitating the clinical progression of patient treatment, working closely with our multidisciplinary team to achieve our goals.
Key Responsibilities- Conduct thorough reviews of patient medical records to determine the appropriateness of admission and continued hospital stay
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement effective care plans
- Communicate with patients, families, and caregivers to ensure that they are informed and engaged in the care process
- Identify and refer quality issues to our Quality Management Program
- Participate in multidisciplinary rounds and contribute to the development of care plans that promote efficient utilization of resources
- Bachelor's degree in Nursing or related field
- Effective decision-making skills and ability to work independently
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
