Job Summary:

Under the supervision of a licensed nurse, performs a variety of non-professional patient care duties for Kaiser Permanente members in a skilled nursing facility. The ideal candidate will be responsible and accountable for assigned tasks, working collaboratively with the healthcare team to provide high-quality patient care.

Key Responsibilities:
  • Collaborate with Licensed Nurses: Provide input to the development and revision of patient care plans, working closely with licensed nurses to ensure effective care delivery.
  • Assist with Data Collection: Gather and record patient data, identifying changes in patient status and reporting them to licensed nurses or supervisors.
  • Implement Care Plans: Set priorities of care with licensed staff, ensuring that patient needs are met in a timely and effective manner.
  • Support Admissions and Transfers: Assist with the admission, discharge, and transfer of patients, ensuring a smooth transition of care.
  • Report Incidents and Concerns: Document and report any incidents, work-related injuries, or safety concerns to supervisors or other appropriate personnel.
  • Practice Infection Control: Demonstrate awareness of and apply infection control principles and standards, ensuring a safe environment for patients and staff.
  • Participate in Quality Improvement: Contribute to quality improvement activities, working collaboratively with the healthcare team to enhance patient care and outcomes.
  • Communicate Effectively: Report pertinent patient data at shift report, and communicate problems or concerns to licensed staff, charge nurses, or supervisors.
Requirements:
  • Education: High School Diploma or equivalent, and completion of a Hospital Aide Training Program or equivalent military training course.
  • Certifications: Nurse Aide Certification (Hawaii) within 3 months of hire, and Basic Life Support certification from the American Heart Association within 1 month of hire.
  • Experience: Minimum one (1) year of experience as a Hospital Aide, or successful completion of a training program in the area of specialization.
