About the Position:

As a Nurse Practitioner - PRN/Part Time Home Risk Assessment Specialist, you will conduct adult/geriatric assessments, medical history reviews, and health education sessions in the comfort of patients' homes. You will work collaboratively with physicians, case managers, social workers, and other healthcare professionals to deliver comprehensive care and support.

Responsibilities:

  • Conduct home risk assessments and medical history reviews
  • Develop and implement personalized care plans
  • Provide health education and support to patients and their families
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Document patient interactions and outcomes

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • 1+ year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
