Job Summary:
Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families.
Responsibilities:
- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
- Conduct home visits, skilled facility visits, and telehealth visits.
Requirements:
- Nurse Practitioner licensure and certification.
- Strong computer skills and familiarity with electronic health/medical record software.
- Ability to travel, valid state driver's license, and adequate automobile insurance coverage.
