Position Title: Addictions Services Case Manager

Employment Type: Full-Time

Compensation: $18.50/Hour

Job Overview:

The Addictions Services Case Manager plays a crucial role in providing comprehensive case management for individuals requiring support in overcoming addiction challenges. This position is integral to the mission of FLACRA, focusing on delivering compassionate and effective services to those in need.

Key Responsibilities:

  • Conduct thorough assessments to determine clients' primary and secondary diagnoses and their need for further counseling.
  • Provide direct therapeutic interventions through individual, family, and group therapy sessions.
  • Manage intake processes and service loads with independent judgment, ensuring high-quality care.
  • Engage in regular meetings with staff for supervision and professional development.
  • Foster positive relationships with community organizations, referral sources, and service agencies.
  • Participate in community outreach and educational initiatives.
  • Assist clients in developing essential interpersonal, problem-solving, and decision-making skills.
  • Demonstrate teamwork and flexibility in a collaborative work environment.
  • Respond effectively to crisis situations to ensure client safety and well-being.
  • Ensure all documentation complies with agency regulations and is completed accurately and promptly.
  • Uphold professionalism and respect in all interactions with clients and colleagues.
  • Perform additional duties as assigned to support agency operations.

Qualifications:

  • Associate's Degree in Health or Human Services is required; Bachelor's Degree is preferred.
  • Valid NYS Driver's License.
  • Strong written and verbal communication skills.
  • Experience in case management is a plus.
  • Ability to manage client visits effectively using personal transportation.
  • Familiarity with the local behavioral health system.
  • Proficient in electronic record-keeping.
