About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Grane Home Health & Hospice. As a Hospice Aide, you will play a vital role in providing personal care services to patients in their homes, supporting them to maintain their dignity and comfort.

Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming, as assigned by the Case Manager.
  • Assist patients with daily living activities, such as feeding, toileting, and transferring.
  • Monitor and report any changes in patient condition to the Case Manager.
  • Participate in quality assessment and performance improvement activities to ensure high-quality care.
  • Collaborate with the hospice team to provide comprehensive care to patients and their families.
Requirements
  • Meet the training requirements of the State.
  • Completed a minimum of seventy-five hours of classroom and supervised practical training.
  • Minimum of six months experience in providing personal care.
  • At least eighteen years of age.
  • Ability to read and follow written instructions and document care given.
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
