Role Objective: The Healthcare Quality Manager will be tasked with leading the quality management initiatives at Families Together Community Health Center (FTOC). This role is crucial in ensuring compliance with all quality standards mandated by regulatory authorities and enhancing patient care outcomes through continuous monitoring and improvement.
Key Responsibilities:
- Develop and execute a thorough quality management strategy for FTOC.
- Evaluate and ensure adherence to quality and regulatory standards.
- Identify areas for enhancement and formulate actionable plans.
- Provide ongoing training to clinical personnel on quality management practices.
- Conduct systematic audits and evaluations of clinical procedures and results.
- Collaborate with the leadership team to meet quality benchmarks and deliver exceptional patient care.
- Manage the Population Value Based Care grant and its components.
- Support the care coordination team in achieving their objectives.
- Facilitate the clinic's certification as a Patient Centered Medical Home (PCMH).
- Act as the primary liaison for regulatory and accreditation agencies.
- Ensure compliance of FTOC's policies and procedures with applicable regulations.
- Train internal stakeholders to promote adherence to established policies.
- Monitor patient satisfaction and implement strategies to enhance patient experiences.
- Maintain comprehensive records of quality management activities and outcomes.
- Build and sustain relationships with external quality improvement organizations.
- Lead quality improvement projects and initiatives.
- Ensure the electronic health record system is aligned with quality measurement needs.
- Work with the risk management team to identify and address patient safety concerns.
- Analyze quality data and prepare reports to share insights with clinical staff and leadership.
- Participate in interdisciplinary committees to advocate for quality and patient safety.
- Align the quality management program with the organization's strategic objectives.
- Lead monthly quality board meetings to present summarized quality metrics and devise action plans.
- Coordinate a quality workgroup to tackle quality issues and improvement projects.
- Stay informed on healthcare quality management trends and communicate updates to staff.
- Foster a culture of continuous improvement and patient-centered care within the clinic.
- Perform additional duties as assigned by the Director of Quality & Risk Management.
Qualifications and Skills:
- Bachelor's degree in healthcare management or a related discipline.
- At least 5 years of experience in healthcare quality management.
- Proficient understanding of quality management principles and methodologies.
- Familiarity with regulatory and accreditation standards for health centers.
- Exceptional communication and interpersonal skills.
- Strong analytical and problem-solving capabilities.
- Ability to collaborate effectively with a diverse team.
- Proficient in Microsoft Office and quality management software.
Additional Skills and Qualifications:
- Outstanding customer service skills.
- Professional verbal and written communication abilities.
- Capacity to communicate effectively with senior management.
- Ability to create summarized reports and presentations.
- Willingness to undergo drug testing if required.
