About the Role

This is a challenging yet rewarding position as a Care Manager II, primarily remote but requiring residency in South Carolina, particularly in York, Florence, Darlington, Williamsburg, or Horry counties.

The successful candidate will play a crucial role in supporting members during transitions of care, coordinating care, educating on discharge plans, and evaluating effectiveness. They will also review medication lists, educate members on pharmacy needs, and counsel them on side effects and treatment protocols.

Key Responsibilities
