About the Role

Advantmed is seeking a highly skilled and compassionate Nurse Practitioner to join our team as a Risk Adjustment Specialist. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.

Key Responsibilities
  • Perform annual wellness visits and health assessments on a population with chronic conditions
  • Deliver patient health education opportunities to promote healthy behaviors and disease management
  • Assist in closing quality care gaps, including screenings and labs, to ensure comprehensive care
  • Collaborate with healthcare teams to enhance care delivery and improve patient outcomes
  • Commit to a minimum of 30 hours per month to ensure consistent and high-quality service
Requirements
  • Must have a valid unencumbered NP License for the state you will be working in
  • Previous In-home Risk Assessment experience is preferred, but not required
  • 3 years of patient care experience in primary care, adult, or geriatric settings, with experience in EMR systems
  • Bilingual skills are a plus, but not required
