Job Title: Aveanna Healthcare Private Duty Nurse LVN

We are seeking a compassionate and skilled Private Duty Nurse LVN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing the continuous care needed to remain at home with their loved ones.

Responsibilities:
  • Implementing physicians' prescribed plans of care
  • Conducting patient assessments and coordinating care
  • Administering prescribed medication, treatments, and therapies
  • Documenting care provided in our system of record
  • Teaching and training family members on health promotion and disease management
  • Accepting and writing verbal orders from physicians as required, and obtaining co-signature of Case Manager on all verbal orders
  • Following up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition
Requirements:
  • Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
