About the Role

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse LVN, you will play a vital role in providing one-on-one care to our patients in their homes, helping them to recover and thrive in the comfort of their own surroundings.

Responsibilities
  • Implement physicians' prescribed plans of care for patients
  • Conduct patient assessments and coordinate care with the healthcare team
  • Administer prescribed medication, treatments, and therapies as needed
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians as required, and obtain co-signature from Case Manager
  • Follow up with patients, families, and healthcare team on client problems, concerns, and changes in client condition
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
