Physical Therapist

We are seeking a skilled Physical Therapist to join our dynamic team at Visiting Nurse Association of Ohio.
Job Summary:The successful candidate will be responsible for providing home care therapy services to individuals, helping to restore function, improve mobility, and relieve pain.
Responsibilities:- Perform initial assessments to determine patient treatment needs.
- Develop and implement plans of care with measurable goals and specific discipline-related treatments.
- Provide direct physical therapy services, including performance of physical therapy procedures and instruction of patients and caregivers.
- Re-evaluate and revise plans of care as needed with physician approval.
- Facilitate total care services by making appropriate referrals to other disciplines.
- Submit all documents in a timely manner.
- Bachelor's Degree in Physical Therapy preferred.
- One year of experience in a supervised healthcare setting.
- Ability to work effectively with diverse cultural, social, and ethnic backgrounds.
- Strong planning and organizational skills.
- Ability to work well under pressure and maintain strict confidentiality.
- Licensed Physical Therapist by the State of Ohio.
- CPR certification required.
- Valid Driver's License.
