Permanent Housing Case Manager

We are seeking a highly motivated and experienced Permanent Housing Case Manager to join our team. In this role, you will be responsible for managing the housing placement process for our clients, ensuring that they receive the necessary support and resources to achieve successful outcomes.
Estimated Salary: $65,000 - $85,000 per annum, based on experience and qualifications.
Key Responsibilities:
- Manage the housing placement process for clients, ensuring successful outcomes.
- Develop and maintain relationships with local housing providers and organizations.
- Provide ongoing support to clients throughout the placement process.
- Collaborate with internal teams to ensure seamless delivery of services.
Requirements:
- Bachelor's degree preferred and/or an Associate degree in Social Work, Sociology, Psychology, Criminal Justice or Human Services.
- Minimum of 2 years of experience working with homeless families to obtain permanent housing.
- Valid New York State driver's license and an insurable driving record.
- Excellent written and verbal communication skills.
- Excellent interpersonal and computer literate in Microsoft-Office.
