Key Responsibilities- Assist residents with daily living activities, such as personal care and medication management.
- Perform daily rounds to ensure policies are being followed and residents are receiving quality care.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Document resident information and medical records accurately and efficiently.
- Ensure compliance with Department of Health Services regulations and company policies.
- Licensed Practical Nurse with current Pennsylvania State License.
- Minimum of two years of nursing experience, with a focus on dementia care and personal care.
- Current CPR certification.
- Basic computer skills and proficiency in electronic medical records.
