Job Title: Field Nurse Case Manager

We are seeking a skilled Field Nurse Case Manager to join our team at Paradigm, a leading accountable specialty care management organization. As a Field Case Manager, you will work in a home-based setting, traveling to doctors' offices, hospitals, and other locations to provide medical case management services for work-related injuries.

Key Responsibilities:
  • Assess, plan, coordinate, implement, and evaluate the medical case management of work-related injuries
  • Work with insurance carriers, medical care providers, attorneys, employers, and employees to monitor the progress of injured workers
  • Closely monitor the progress of injured workers and report results back to employers and insurance carriers
Requirements:
  • Current, unencumbered Registered Nurse (RN) license in NJ and NY
  • Experience in case management of workers comp cases, preferred
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
  • Ability to operate autonomously with minimal oversight
  • Skilled at patient education
  • Valid driver's license, and good driving history
