Homelessness represents a significant social and public health challenge in California. Illumination Foundation (IF) is an expanding non-profit organization committed to breaking the cycle of homelessness by offering specialized, multidisciplinary services in our recuperative care centers, emergency shelters, housing assistance, and programs for children and families. IF currently operates over 13 facilities with more than 22 micro-communities throughout Orange County, Los Angeles County, and the Inland Empire.

Position Summary

The main objective of the Intensive Case Management Services (ICMS) program is to provide sustainable, affordable housing connected to essential services for homeless individuals who frequently utilize the Department of Health Services system. The ICMS Case Manager collaborates directly with program participants to develop, coordinate, and arrange treatment services. This role aims to enhance the well-being of homeless individuals by facilitating stable housing, creating comprehensive treatment plans, and linking them to medical care, mental health support, substance use treatment, and other necessary services. After securing permanent housing, the ICMS Case Manager assists clients in maintaining their housing stability. This position demands a commitment to confidentiality and the ability to manage sensitive information with professionalism.

The compensation for this role is $26.00 per hour, based on experience.

Key Responsibilities

Intensive Case Management Services:

  • Manage a caseload of 15-20 clients.
  • Evaluate current clients for eligibility and refer them to the program as needed.
  • Assess incoming referrals for eligibility.
  • Conduct thorough assessments to identify case management service needs.
  • Create personalized treatment plans with measurable goals and support participants in achieving their objectives.
  • Assist clients in identifying and securing necessary resources.
  • Advocate for clients to ensure they receive maximum benefits and resources to promote self-sufficiency.
  • Build relationships with representatives from Illumination Foundation and other agencies to help individuals access services such as housing, mental health care, medical treatment, financial aid, and legal support.
  • Conduct regular home visits to assess needs and connect clients with community services.
  • Engage with unhoused clients weekly to evaluate needs and refer them to appropriate services.
  • Participate in community and collaborative meetings relevant to ICMS.
  • Assist in preparing Section 8 housing applications and related documentation.
  • Maintain communication and foster positive relationships with landlords to support client housing retention.
  • Network and collaborate with housing providers, landlords, and property managers to expand housing options for the program, including temporary and permanent solutions.

Administrative Duties:

  • Coordinate, prepare, and maintain required documentation and records promptly and accurately.
  • Engage in daily debrief sessions, weekly supervision, and department-wide meetings as required.
  • Participate in training and development opportunities as deemed appropriate by the supervisor.
  • Stay informed about agency policies and procedures as well as ICMS program contracts.
  • Maintain client records in compliance with HIPAA and uphold all confidentiality standards.
  • Contribute to the overall evaluation of program services and assist in data collection for reporting by keeping accurate records in participant files.

Qualifications

Required:

  • Bachelor's degree in Social Services, Healthcare, or a related field; or equivalent combination of education and experience.
  • Experience in homeless services, case management, mental health, or permanent supportive housing.
  • Valid California driver's license.
  • Experience in non-profit housing or housing for individuals with disabilities and chronic health conditions.
  • At least one year of experience working with at-risk or homeless populations (preferred).
  • Familiarity with VI-SPDAT.

Preferred:

  • Bilingual in English and Spanish.
  • Proficiency in Google Suite (Mail, Docs, Sheets, Calendar) and Microsoft Office Suite (Outlook, Word, Excel).
