Position Overview:

Key Responsibilities:
  • The Primary Care Physician (PCP) is accountable for patient outcomes, delivering high-quality clinical care, and ensuring cost-effectiveness for a panel of up to 450 patients. By fostering trusting relationships, the PCP significantly influences patient outcomes and encourages positive behavior changes.

Health Coaching:

The PCP serves as a health coach, guiding patients in setting and achieving both short-term and long-term health objectives. This partnership involves regular follow-ups to ensure progress toward improved health.

  • The PCP prioritizes and simplifies care processes, making behavior change actionable for patients and leading care teams toward performance goals.

As an outcomes-focused, value-based organization, we regularly assess metrics such as patient admissions per thousand, appointment utilization, patient experience scores, clinical gap closures, and medical cost measures to support PCP success and partnership status.

Cultural Leadership:Culture is paramount in our medical centers. As leaders, PCPs are expected to foster a positive environment characterized by love, accountability, and passion alongside center leadership.

The PCP collaborates with managers and medical directors to ensure adherence to guidelines and actively participates in risk and quality management initiatives, clinical meetings, and other necessary gatherings that promote patient health and organizational objectives.

Patient Care:The PCP independently manages care for patients with both acute and chronic conditions, particularly in the older adult demographic. They engage with hospitalists when their patients are hospitalized, regardless of the hospitalist's affiliation.

The PCP is responsible for comprehensive patient assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs.

The PCP leads a care team that includes a medical assistant, care facilitator, and care coordinator, ensuring effective management of patients who can visit the office. For those unable to attend in-person appointments, the PCP collaborates with transitional care teams and other resources as needed.

The PCP plays an active role in center management and provides coverage for colleagues as necessary.

Availability for patients is crucial; PCPs are encouraged to be accessible for walk-in visits and to respond promptly to calls and messages.

Commitment to Quality:The PCP remains current with evidence-based medicine while recognizing the significant impact of lifestyle and nutrition on health outcomes. Physician leadership is essential for quality healthcare, and the PCP is expected to continuously enhance their leadership skills for the benefit of patients, their team, and the organization.

Our patients deserve the highest quality of care, which requires a commitment to achieving superior outcomes and quality measures. PCPs are also encouraged to seek systematic improvements in their medical practice and center operations.

Qualifications:MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required. Candidates must possess or be eligible for a State Medical License and preferably hold board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Proficiency in Microsoft Office Suite and the ability to travel locally and nationally for training and recruitment purposes are also necessary. Fluency in English, both verbal and written, is required, with additional language proficiency considered an asset.
