Key Responsibilities:

  • Oversee and conduct resident assessments and care planning in compliance with state and federal regulations, ensuring optimal resident functioning.
  • Coordinate the MDS, CAAS, and care planning processes for new admissions.
  • Perform thorough checks at the start of each month for all Medicare, Managed Care, and outpatient therapies.
  • Monitor and report any changes in residents' physical and emotional health to the nursing staff.
  • Manage and update necessary documentation weekly.
  • Establish care plan timelines within seven days of MDS completion or quarterly review, adhering to regulations.
  • Ensure nursing documentation aligns with the care plan during treatment administration.
  • Record all information gathered during the assessment process.
  • Set Assessment Reference Dates (ARD) for residents and communicate these with the interdisciplinary team.

Qualifications:

  • Graduate from an accredited LPN/RN program.
  • Valid LPN/RN license in Illinois.
  • Minimum of two years of experience in MDS.
  • Recent clinical experience and specialized skills in geriatrics.
  • Willingness to work flexible hours, including evenings, weekends, and holidays.
  • Passion for working with the senior population.
