Key Responsibilities:
- Oversee and conduct resident assessments and care planning in compliance with state and federal regulations, ensuring optimal resident functioning.
- Coordinate the MDS, CAAS, and care planning processes for new admissions.
- Perform thorough checks at the start of each month for all Medicare, Managed Care, and outpatient therapies.
- Monitor and report any changes in residents' physical and emotional health to the nursing staff.
- Manage and update necessary documentation weekly.
- Establish care plan timelines within seven days of MDS completion or quarterly review, adhering to regulations.
- Ensure nursing documentation aligns with the care plan during treatment administration.
- Record all information gathered during the assessment process.
- Set Assessment Reference Dates (ARD) for residents and communicate these with the interdisciplinary team.
Qualifications:
- Graduate from an accredited LPN/RN program.
- Valid LPN/RN license in Illinois.
- Minimum of two years of experience in MDS.
- Recent clinical experience and specialized skills in geriatrics.
- Willingness to work flexible hours, including evenings, weekends, and holidays.
- Passion for working with the senior population.
