Job Title: Home Health Occupational Therapist Assistant Job Summary: LHC Group is seeking a highly skilled and dedicated Home Health Occupational Therapist Assistant to join our team. As a Home Health Occupational Therapist Assistant, you will play a vital role in providing occupational therapy to patients in their homes, helping them to restore their physical abilities and improve their quality of life. Responsibilities: * Under the supervision of an Occupational Therapist, you will provide occupational therapy to patients in their homes, following a treatment plan that is tailored to their individual needs. * You will instruct and aid patients in evidence-based treatment, using your skills and knowledge to help them achieve their goals. * You will observe, record, and report to the supervising OT, nurse supervisor, and/or physician the patient's response to treatment and changes in the patient's condition. * You will coordinate care with other members of the healthcare team as appropriate. * You will instruct patient, family, caregiver, and other members of the healthcare team in the areas of therapy within the scope of the occupational therapy assistant. * You will visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed. * You will complete a progress note for each visit and submit progress notes per LHC policy. * You will adhere to LHC Therapy Standards of Practice in provision of service to all patients. * You will participate in staff conferences and committees as requested by the agency. * You will have the flexibility to work a variety of schedules, including weekends and holidays, to ensure that we provide care to our patients as needed. Requirements: * Current OTA licensure in the state of employment * Current CPR certification * Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation * Additional state requirements may apply, please see below Additional State Requirements: * AL: Must have two (2) years experience practicing as an Occupational Therapist * CT: Must be certified by the American Occupational Therapy Association * District of Columbia: Not allowed to practice * IN: Must be certified by a national therapy association * LA: Must have a minimum of two (2) years experience as a licensed Occupational Therapist * NY: Must be certified and registered with the state of New York * SC: Must be certified and in good standing with the National Board for Certification in Occupational Therapy * AR, AZ, CA, CO, DE, FL, GA, ID, IL, KY, MA, MD, MI, MS, MN, MO, NC, NH, NJ, NM, NV, OH, OK, OR, PA, RI, TN, TX, VA, WA, WV, WI: No additional state specific requirements About LHC Group: LHC Group is the preferred post-acute care partner for hospitals, physicians, and families nationwide.
