About the Role:
We are seeking a dedicated and compassionate Community Support Specialist to join our dynamic team at Community Partners, Inc.
As a Community Support Specialist, you will play a vital role in establishing connections and providing essential resources for adults facing severe and persistent mental illness within our community.
You will demonstrate a passion for helping others and a commitment to collaborative, multidisciplinary teamwork.

Key Responsibilities:
- Utilize your excellent communication skills to establish connections and provide resources to individuals in need.
- Collaborate with our multidisciplinary team to provide comprehensive support services.
- Develop and maintain a thorough understanding of community resources and services.
- Provide education and support to individuals and their families to promote independence and well-being.
Requirements:
- Bachelor's degree in the Human Services field
- Excellent oral and written communication skills
- Ability to utilize technology provided to perform requirements of the job
- Hold a valid driver's license and proof of vehicle liability coverage
