Job Title: Hospice Intake, LPN

Concordia Lutheran Ministries and Affiliates is seeking a compassionate and skilled LPN to join our team as a Hospice Intake Coordinator. This is a full-time position working Monday through Friday with a weekend rotation (every third) and is based out of the Valencia Office.

Job Summary:

The LPN Intake Coordinator is responsible for facilitating patient/family education, evaluation, and/or the admission of patients into hospice care and/or associated services. This role requires strong assessment, problem-solving, and decision-making skills, as well as excellent organizational and communication skills.

Responsibilities:
  1. Discusses hospice information with referral sources to assess, plan, and implement timely transition of patients to hospice care.
  2. Works closely with the scheduler to meet referral source and patient/family expectations regarding Reflection of Care (ROC), evaluation, and/or admission.
  3. Completes referral information appropriately.
  4. Obtains necessary information for patient admission or ROC, including orders, demographic information, and health records.
  5. Obtains insurance authorizations in a timely manner.
  6. Gathers information to ensure the safety of patients, families, and employees.
  7. Adheres to regulatory compliance activities and processes.
  8. Facilitates necessary data collection and entry of patient information for physicians and Interdisciplinary Group (IDG) members; arranges the necessary IDG members' involvement related to the admission.
  9. Communicates accurately with coding, authorization, and Inpatient Units to ensure regulatory management of hospice criteria, NOEs, and eligibility.
  10. Provides information to Patient Care Coordinators (PCC) to ensure management of medications.
  11. Completes all computer and/or paper documentation requirements within the specified timeframes.
  12. Participates in required on-call responsibilities.
