Job Summary

Telecare Corporation is seeking a skilled LVN (Licensed Vocational Nurse) to join our team as an On Call Behavioral Health Specialist at Redwood Place Mental Health Rehabilitation Center. As a key member of our multidisciplinary team, you will provide recovery-focused services to adult residents with severe mental illness and developmental disabilities.

Responsibilities
  • Implement and supervise assigned positions within the nursing department on designated shifts.
  • Provide program planning, development, review, and assessment of clinical and administrative activities.
  • Direct, coordinate, and supervise a multidisciplinary team, assigning staff to work with clients based on their individual needs and skills.
  • Provide nursing care, assist with welcoming, admissions, and discharge processes, and collect data relevant to member care.
  • Develop and implement care plans, demonstrate knowledge of crisis management and intervention, and administer medication and treatments as prescribed.
  • Coordinate and participate in counting medications, give reports, and notify prescribers and nursing supervisors of changes in behavior or condition.
Requirements
  • Graduation from an accredited LVN or LPT program and current licensure.
  • Current CPR/BLS certificate.
  • Two years of LVN/LPT/LPN experience in a healthcare environment.
  • One year of supervisory experience or completion of Telecare assigned Supervisory training within 12 months.
  • Crisis Prevention Institute (CPI) certification on date of employment or within 60 days.
Preferred Qualifications
  • Previous experience working with the population served and/or in a similar program setting.
  • Understanding of psychiatric illness and its treatment, and knowledge of psychotropic medication.
Skills
  • Ability to review and implement Recovery/Treatment plans.
  • Ability to administer oral and injectable medications.
  • Ability to provide input by reporting observations and concerns and ask appropriate questions.
  • Ability to implement crisis de-escalation techniques.
Physical Demands

The physical demands of this job are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is occasionally required to sit, squat, kneel, twist, push, pull, and lift items weighing 25 pounds or less. The position requires manual dexterity, repetition, and Hazardous Exposure (Blood Borne Pathogens, Hospital Waste, Chemical & Infectious).
