Key Responsibilities:
  • Schedule Management: Manage patient schedules, ensuring timely and efficient care delivery. Coordinate with field staff, patients, and physicians to maintain proper care coordination and continuity of care.
  • Office and Staff Management: Assist with day-to-day office and staff management, including editing schedules, updating agent unavailability, and maintaining the client hospitalization log.
  • Communication and Coordination: Serve as a liaison between patients, field staff, and office staff, ensuring seamless communication and coordination of care.
  • Scheduling and Reporting: Complete requested schedules, run scheduling reports, and prepare weekly agent schedules for approval.
  • Quality Assurance: Verify visit paper notes and assist with internal transfers of patients between branch offices.
Requirements:
  • Licensure: Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which you practice.
  • Experience: Have at least 1 year of home health experience.
  • Skills: Possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.
