Job Description

We are seeking a highly skilled Family Health Provider to provide primary care to our patients in the clinic. The ideal candidate is an example of our organization's mission and has strong interpersonal skills. This position requires regular attendance at meetings and the successful candidate must be capable of both professional and personal time management.

Key Responsibilities:
  • Monitor and assess patient health status and develop a plan for treatment.
  • Provide primary care, including routine physical exams, minor acute illnesses, health assessments, and basic wellness education.
  • Maintain detailed patient records and communicate with other healthcare professionals as appropriate.
  • Perform minor office surgeries and procedures under the supervision of another doctor.
  • Manage care for chronically ill patients to help achieve positive outcomes.
  • Manage care for obese patients to help achieve positive outcomes.
  • Work as part of a team.

Requirements:
  • FNP Primary Care: 1 year (Required)
  • Certified Nurse Practitioner (Required)
  • DEA License

This is a full-time position with a competitive salary range of $95,000.00 - $105,000.00 per year. The standard shift is day shift, Monday to Friday, with no weekends. The work setting is in a clinic, in-person.
