Responsibilities:- Coordinate patient care from referral to post-transplant, ensuring seamless continuity of care.
- Collaborate with patients, families, and staff to provide education, support, and advocacy.
- Work closely with the transplant team to ensure timely and effective patient care.
- Act as a liaison between internal and external customers, providing expert consultation and guidance.
- Support the ventricular assist device program as defined in the essential job functions.
- Registered Nurse with a Bachelor Degree in Nursing (or related field) and current Texas licensure.
- Minimum of 3 years of surgical/medical experience.
