Nurse Care Coordinator

Job Summary:
The Home Health Registered Nurse (RN) is responsible for providing high-quality patient care in the comfort of their own homes. This role requires a skilled and compassionate nurse who can work independently and as part of a team to deliver exceptional patient outcomes.
Key Responsibilities:
- Perform initial home care patient visits and re-evaluate patient needs and progress on a regular basis.
- Initiate the plan of care under physician orders and coordinate patient services.
- Supervise LPNs and HHAs and educate patients and caregivers on disease processes, medications, and individualized treatment plans.
- Monitor reactions and patient progress, and notify the physician and other personnel of any changes in the patient's condition.
- Update clinical records according to policy and procedures, and adhere to Medicaid and Medicare regulations.
Qualifications:
- Graduate of an approved school of professional nursing.
- Currently licensed in this Agency's practicing State.
- Minimum of one year of nursing experience as a RN.
