Sub-Acute Therapy Position

As a Physical Therapist with Preferred Therapy Solutions, your primary responsibility will be to provide comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting. You'll work collaboratively with other rehab therapists and nursing staff to ensure the highest quality care for our residents. Some key responsibilities include:
• Providing physical therapy services to patients as part of an interdisciplinary rehabilitation team
• Conducting thorough assessments, developing and implementing individualized treatment plans, and evaluating patient progress
• Communicating effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Maintaining accurate and timely documentation of patient records, assessments, and treatment plans
• Participating in continuous quality improvement initiatives to enhance patient outcomes and satisfaction
• Adhering to all applicable laws, regulations, and professional standards for physical therapy practice
