Job Title: RN - Clinical Manager

About the Role:

As a Clinical Manager, you will be responsible for overseeing the clinical operations of our home health care team, ensuring that our patients receive the best possible care and support. Your key responsibilities will include:

  • Conducting quarterly quality improvement meetings to identify trends and implement improvement plans
  • Collaborating with our Branch Director to implement quality improvement activities and corrective action plans
  • Providing feedback to Operations management on necessary changes and revisions relating to care provision
  • Ensuring all clinical staff complete orientation and required in-services
  • Overseeing and responding to external and internal surveys and audits within assigned areas
  • Acting as a resource and having a working knowledge of payer requirements and documentation guidelines
  • Completing documentation and chart reviews to identify variances in standardized care and care processes
  • Holding case conferences to ensure oversight of care, coordination of services, and standards are met
  • Ensuring documentation standards are met per company standards and to support quality standards, coverage, and reimbursement
  • Ensuring care is coordinated with all involved in patient care
  • Identifying variances in standardized care and care processes to identify areas for improved performance related to patient care
  • Ensuring compliance with regulatory, payer, and company requirements
  • Facilitating the development of performance improvement action plans based on available data
  • Accomplishing staff results by communicating job expectations, planning, monitoring, and appraising job results, coaching, counseling, and disciplining employees, developing, coordinating, and enforcing systems, policies, procedures, and productivity standards
  • Overseeing and directing the selection and hiring of clinical staff
  • Conducting interviews and making hiring decisions
  • Ensuring staff competency and performance
  • Ensuring assigned staff are evaluated fairly and timely in accordance with company performance evaluation procedures
  • Ensuring direct, on-site supervision for all staff per company requirements
  • Responsible for achieving organizational performance and retention goals among clinical staff
  • Serving as a resource in assisting the field staff with problem-solving to provide the most appropriate care for the patient
  • Serving as a coach, mentor, and role model to all professional and direct-care staff
  • Ensuring adequate numbers of preceptor staff to provide development and mentoring of staff clinicians and direct-care staff
  • At least one preceptor clinician is trained to provide back-up for the clinical manager position
Qualifications:

To be successful in this role, you will need:

  • Bachelor's Degree in Nursing or Physical Therapy from an accredited school of nursing or therapy
  • Licensed Registered Nurse (RN) or Physical Therapist (PT) in good standing and currently licensed by the State
  • Less than two years clinical experience, and at least one year in related management/supervisory role in home health care
  • Current CPR certification
  • Ability to use company documentation systems
  • Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
  • Ability to work independently as well as part of a team
  • Capable of working responsibly with highly confidential information
