Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team at Northwell Health. As a Home Care Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Conduct thorough patient assessments, identifying individualized care needs and developing personalized plans of care.
  • Collaborate with physicians and other healthcare professionals to coordinate patient care, ensuring seamless transitions and optimal outcomes.
  • Provide direct patient care, including administering medications, performing treatments, and educating patients and families on self-care and disease management.
  • Document patient progress, identifying areas for improvement and implementing evidence-based interventions to enhance patient outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies that promote patient safety and satisfaction.
  • Communicate effectively with patients, families, and healthcare team members, ensuring clear and timely exchange of information.
Requirements
  • Graduate of an accredited School of Nursing, with a current license to practice as a Registered Professional Nurse in New York State.
  • Specialized certifications, such as PALS, may be required depending on the area of assignment.
  • Valid Driver's License in New York State may be required for certain assignments.
Preferred Qualifications
  • Bachelor's Degree in Nursing, with a strong foundation in nursing theory and practice.
  • Experience working in home care or a related setting, with a proven track record of delivering high-quality patient care.
