We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our team at Della Infotech, Inc. in New London, NH.
The estimated salary for this position ranges from $60,000 to $80,000 per year.
About the Role:
This is an excellent opportunity for a licensed nurse practitioner to join our primary care team and contribute to the delivery of high-quality patient care. You will be responsible for assessing, diagnosing, and treating patients in a variety of settings, including clinics and community-based programs.
Key Responsibilities:
- Conduct comprehensive health assessments and develop individualized treatment plans.
- Order and interpret diagnostic tests, prescribe medications, and perform procedures as needed.
- Collaborate with healthcare teams to coordinate care and ensure seamless transitions.
- Provide education and support to patients and their families on health-related topics and self-care techniques.
Requirements:
- Master's degree in Nursing or related field.
- Current licensure as a Nurse Practitioner (NP) in the state of New Hampshire.
- 1+ year of experience in primary care setting.
- Clinical office experience and phone triage experience required.
- EPIC EMR experience highly preferred.
- Previous travel experience preferred.
