At D-H Lebanon-MHMH, we are seeking a highly skilled Clinical Nursing Specialist to join our team.

Job Summary:

We are looking for a dedicated and compassionate Clinical Nursing Specialist who will work closely with our interdisciplinary team to provide exceptional patient care. As a key member of our nursing staff, you will be responsible for assessing patients' needs, developing individualized care plans, and implementing evidence-based interventions to achieve optimal outcomes.

Responsibilities:
  • Assess patients' physical and psychosocial needs, identifying areas for improvement and developing targeted interventions.
  • Develop and implement individualized care plans in collaboration with multidisciplinary teams.
  • Provide education and support to patients and families, promoting health literacy and self-management skills.
  • Collaborate with healthcare providers to coordinate care and ensure seamless transitions between settings.
  • Maintain accurate and timely documentation of patient interactions and progress.
Requirements:
  • A minimum of one year of experience in nursing, preferably in an acute or critical care setting.
  • Current RN licensure in the State of New Hampshire required.
  • Current BCLS Certification required.
Compensation and Benefits:

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as a comprehensive benefits package that includes medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
