Nurse Care Manager

We are seeking a skilled Licensed Practical Nurse to join our team at SR COMPANIES LLC. As a key member of our care team, you will be responsible for providing high-quality nursing care to our residents and their families.
Key Responsibilities- Assist in the admission process of new residents, ensuring a smooth transition to our community.
- Perform various nursing procedures to promote the comfort and well-being of residents, including assisting with activities of daily living as needed.
- Administer medications as prescribed by healthcare providers.
- Supervise non-licensed nursing staff and participate in their evaluation and development.
- Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided.
- Conduct regular rounds to assess resident needs and provide timely interventions.
- Document resident care and services provided in accordance with regulatory requirements and company policies.
- Collaborate with the care team to identify and address resident concerns and issues in a timely and effective manner.
- Participate in training and education programs to enhance nursing skills and knowledge.
- Valid Florida nursing license.
- One to two years of experience in a healthcare setting, preferably in a long-term care or assisted living environment.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
- Ability to work independently and as part of a team, with a focus on providing high-quality care and services.
- Basic computer skills and knowledge of electronic medical records.
The physical demands of this role include standing, walking, and lifting up to 50 pounds. The work environment is fast-paced and requires the ability to multitask and prioritize tasks effectively.
