Social Services Specialist

As a Social Services Specialist, you will play a vital role in enriching lives by working with patients, residents, and their families to identify strengths, social, emotional, and mental health needs. Your goal will be to provide patients and residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Key Responsibilities:- Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
- Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
- Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
- Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
- Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
