About the Role

We are seeking a highly skilled Senior Case Coordinator to join our team at St. Elizabeth's Medical Center, supporting individuals with developmental disabilities and their families in Riverside Adult West area. The ideal candidate will have experience in case management, intellectual/developmental disabilities, social work, or related field.

Responsibilities:

  • Coordinate services and supports for consumers in accordance with the Lanterman Developmental Disabilities Services Act.
  • Utilize Person-Centered Planning strategies to create and execute Individual Program Plans (IPPs) for consumers.
  • Provide advocacy services, attend meetings, and prepare paperwork as necessary.
  • Seek out and utilize generic resources on behalf of consumers and their families.
  • Complete crisis intervention, document activities, and report incidents timely.
  • Ensure Medicaid Waiver standards are maintained, and documentation is completed on time.
  • Arrange placements in residential facilities, day programs, and other training programs as clinically indicated.
  • Complete facility audits, identify problems, and refer to Quality Assurance and Resource Development teams.
  • Maintain accurate records, update consumer file information, and enter Target Case Management notes.

Requirements:

  • Bachelor's degree from an accredited college or university and one year of experience in case management, intellectual/developmental disabilities, social work, or related field.
  • Ability to establish and maintain effective working relationships with others.
  • Good attendance, punctuality, verbal, and written communication skills.
  • Fully utilizing an automobile, valid California driver's license, liability insurance, or independent transportation.
