Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN Hospice Nurse Practitioner. In this role, you will work closely with our interdisciplinary team to provide high-quality care to patients in need of hospice services.
Responsibilities- Conduct Face-to-Face (F2F) encounters with patients to gather clinical information and support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Perform physical assessments and provide clinical findings to the physician.
- Make recommendations to the IDG based on F2F assessment findings.
- Notify the hospice agency when unable to complete a F2F visit according to schedule.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
