We are seeking a dedicated Clinical Pharmacist III to deliver specialized clinical pharmacy services within a healthcare environment. This role encompasses comprehensive medication therapy management, provision of drug information, and patient education. The successful candidate will collaborate closely with healthcare professionals to guarantee the safe and effective utilization of medications in patient care.
Key Responsibilities:
- Deliver clinical pharmacy services, including medication therapy management, drug information, and patient education to both patients and healthcare providers.
- Work in partnership with physicians, nurses, and other healthcare professionals to enhance medication therapy for patients.
- Analyze and interpret patient medical histories, laboratory results, and other pertinent information to determine suitable medication therapy.
- Oversee and evaluate patient medication therapy for efficacy and adverse effects, providing recommendations to healthcare providers as necessary.
- Engage in medication safety initiatives, including the identification and reporting of adverse drug events and medication errors.
- Offer education and training to healthcare providers and patients on medication-related subjects.
- Develop and implement protocols and guidelines for medication use tailored to specific patient populations or disease states.
- Act as a drug information resource for healthcare providers, patients, and caregivers.
- Participate in formulary management, drug utilization review, and other pharmacy-related activities.
- Contribute to research and quality improvement initiatives associated with medication therapy.

Qualifications:
- Doctor of Pharmacy degree from an accredited institution.
- Completion of a PGY-2 residency in a clinical pharmacy specialty or equivalent experience.
- Board certification in a clinical pharmacy specialty (e.g., BCPS, BCCCP, BCOP, etc.) is preferred.
- Active pharmacy license in the state of practice.
- Strong clinical expertise in medication therapy management, drug information, and patient education.
- Excellent communication and interpersonal skills.
- Ability to collaborate effectively with healthcare providers as part of an interdisciplinary team.
- Strong analytical and problem-solving capabilities.
- Ability to prioritize and manage multiple tasks and projects efficiently.
