Job Summary

We are seeking a highly skilled Physical Therapist to join our Outpatient Rehabilitation team at Luminis Health. As a Physical Therapist, you will play a critical role in evaluating patients and providing therapeutic interventions to help them achieve optimal physical function and mobility.

Key Responsibilities
  • Evaluate patients to determine their physical therapy needs and develop effective treatment plans to address their rehabilitation goals.
  • Conduct physical therapy evaluations and assessments to identify areas of improvement and develop targeted interventions.
  • Provide technical supervision and direction to Physical Therapist Assistants, Rehab Aides, students, and volunteers to ensure high-quality patient care.
  • Collaborate with physicians, other clinicians, and interdisciplinary teams to ensure comprehensive patient care and to identify opportunities for physical therapy services.
  • Participate in departmental in-service activities, continuing education seminars, and process improvement initiatives to stay current with best practices and to contribute to the growth and development of the team.
  • Maintain accurate and detailed records of patient progress, treatment plans, and outcomes to ensure compliance with departmental policies and procedures.
  • Comply with all departmental policies, procedures, and standards of practice to ensure a safe and effective work environment.
Requirements
  • Higher Education Level required for PT Licensure
  • Outpatient rehabilitation or related experience
  • Physical Therapist licensure and CPR certification
Working Conditions

This position requires the ability to work in a fast-paced environment and to adapt to changing priorities and deadlines. The physical demands of this position include heavy lifting, bending, and standing for extended periods.
