Nurse Care Specialist

We are seeking a skilled Registered Nurse to join our team at Corewell Health. As a Nurse Care Specialist, you will be responsible for providing high-quality patient care and collaborating with our healthcare team to ensure the best possible outcomes.
Key Responsibilities- Assess and monitor patient needs, developing and implementing individualized care plans
- Administer medications and communicate with patients, families, and healthcare team members
- Coordinate care and act as a patient advocate, ensuring seamless transitions and effective communication
- Educate patients and family members on health conditions and treatment plans
- Report patient condition and outcomes to physicians, participating in process improvement activities to ensure quality patient care
- Associate's Degree in Nursing required, Bachelor's Degree preferred
- 1 year of relevant acute care, emergency, or rehab facility experience
- Registered Nurse (RN) license in the State of Michigan
- Valid Michigan Driver's License
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross
The ideal candidate will be able to perform varied physical demands, including lifting, carrying, and standing.
