Job Summary

We are seeking a skilled Case Manager to join our team at Oaklawn. As a Case Manager, you will play a critical role in helping youth achieve their goals and live fulfilling lives.

Key Responsibilities

  • Assess the needs of eligible individuals and develop personalized care plans
  • Referral and related activities to advocate and assist individuals obtain needed services
  • Monitor and follow up on care plans
  • Evaluate the impact of treatment plans on recovery
  • Interact frequently with treatment teams, including residential therapists, physicians, and community agencies
  • Linkage and networking with DCS, court, schools, healthcare providers, employers, and other community agencies
  • Provide integration and ongoing assistance to youth and families to help sustain within the community/least restrictive environment
  • Accountable for the flow of information within a multidisciplinary team setting

Requirements

  • BA/BS in social service or a human services-related field required
  • Two years' experience in a social service setting preferred
  • Valid driver's license and proof of insurance

As a Case Manager, you will be responsible for helping youth achieve their goals and live fulfilling lives. You will work closely with treatment teams, including residential therapists, physicians, and community agencies, to provide comprehensive care and support.
