Job Summary

We are seeking an experienced Nurse Clinical Manager to lead our interdisciplinary team, ensuring the delivery of high-quality care and fostering staff development and operational efficiency. This position requires a compassionate leader committed to upholding our mission, vision, and values.

Key Responsibilities:
  • Patient Care Coordination
    • Oversight of scheduling and adherence to individualized care plans.
    • Conducting team meetings and patient care conferences.
    • Collaboration with patients, families, physicians, and care teams for optimal outcomes.
  • Personnel Management
    • Supervising and evaluating clinical staff performance.
    • Assisting in recruitment, onboarding, and orientation of new team members.
    • Conducting performance evaluations and staff development initiatives.
  • Operational Oversight
    • Managing caseloads and productivity.
    • Ensuring compliance with regulatory standards.
    • Participation in administrative on-call rotations and insurance authorizations.
  • Leadership
    • Promoting a collaborative work environment.
    • Facilitating professional growth and team cohesion.
    • Acting as a resource for clinical problem-solving and decision-making.
  • Performance Improvement
    • Collaborating with Quality Improvement to enhance patient outcomes.
    • Ensuring compliance with Medicare and PA Home Health Licensure standards.
  • Customer Service
    • Fostering positive relationships with internal and external stakeholders.
    • Maintaining accountability for patient and staff satisfaction.

Qualifications:

  • Current PA RN license, BSN preferred.
  • Minimum 3 years of clinical experience in Home Health.
  • Strong leadership skills.
  • Valid driver's license with reliable transportation.
  • CPR certification.
  • Strong leadership, communication, and organizational skills.

Estimated Salary Range: $90,000 - $110,000 per year, depending on experience.
