About the RoleThe Nurse Case Manager will play a critical role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. This includes coordinating efforts within the rehabilitation team, collaborating with the rehabilitation program director and facility case management director, and ensuring the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences.
Responsibilities- Coordinate and direct care specific to the needs of each rehabilitation patient
- Collaborate with the rehabilitation program director and facility case management director regarding departmental functions
- Ensure the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences
- Facilitate the involvement of the patient throughout the rehabilitation process
- Document the findings of the discharge planning evaluation and psychosocial assessments
- Communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver
We are looking for a candidate with an Associate Degree or Bachelor's Degree in Nursing and 2+ years of experience in case management or 3+ years of experience in clinical nursing.
