Aveanna Healthcare is a leading provider of home health services to medically fragile children and adults. We are seeking a skilled Private Duty Nurse LVN to join our team.

About the Role

As a Private Duty Nurse LVN, you will provide one-on-one care to our patients in their homes, assisting with daily living activities and administering medications as prescribed. You will work closely with our patients, their families, and our healthcare team to ensure the highest level of care and support.

Responsibilities
  • Implement and manage patient care plans in collaboration with our healthcare team
  • Conduct patient assessments and coordinate care with other healthcare professionals
  • Administer medications and treatments as prescribed by physicians
  • Document patient care and progress in our electronic health record system
  • Provide education and support to patients and their families on disease management and self-care
  • Collaborate with our healthcare team to ensure seamless care transitions and coordination
Requirements
  • Active, unencumbered LVN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
