About the Position

New Avenues to Independence, Inc. is seeking a highly skilled and compassionate Care Coordinator to join our team.

The estimated salary for this position is $55,000 - $65,000 per year, depending on experience.

Job Description

The QIDP (Qualified Intellectual Disability Professional) will be responsible for coordinating programs within assigned homes, developing and implementing Individual Plans (IPs) that promote self-determination, development, and independence of those we serve.

This role requires ensuring all systems and practices meet regulatory agency requirements, assisting staff in performing program services plan responsibilities, and jointly supervising group home staff with the Residential Coordinator.

Key Responsibilities

  • Develop Comprehensive IP: Collaborate with the team to create comprehensive IPs tailored to individual needs, monitor active treatment programs, and ensure proper documentation.
  • Facilitate Admissions and Discharges: Participate in individual admissions, discharges, and service substitutions with residential sites.
  • Behavior Support Programs: Develop and implement behavior support programs in conjunction with the behavior support committee.
  • Advocacy: Advocate for individual rights and needs.
  • Regulatory Compliance: Maintain current knowledge of regulatory compliance agencies and adhere to their requirements.
  • Communication: Communicate effectively with individuals involved in an individual's life.
  • MUI Process: Lead in MUI (Maltreatment, Unnecessary Restraint, or Inhumane Treatment) process.
  • Portal Maintenance: Maintain and enter information into the DODD portal.
  • QIDP Absence: Assist with QIDP responsibilities in their absence as directed by the Residential QIDP Manager.
  • Survey Preparation: Assume responsibilities in survey and inspection preparation, focusing on daily compliance with expectations.
  • Staff Supervision: Coordinate with the Core Team to supervise Direct Support Professionals regarding IPs, skill building, and behavioral supports.
  • Recruitment and Training: Participate in recruiting, orienting, and training Direct Support Professionals.
  • Evaluation: Evaluate staff on professional interventions used in skill building, active treatment, and IP implementation.
  • On-Call Responsibility: Assume on-call responsibility according to agency practices and expectations.

Requirements

Education and Experience: Bachelor's or Graduate degree from an accredited college or university; OR hold professional license or certification issued by the Ohio board of psychology; the state medical board of Ohio, or the Ohio counselor, social worker, and marriage and family therapist; OR hold a certificate to practice as a certified Ohio behavior analyst pursuant to section 4783.04 of the Revised Code.

Certificates, Licenses, Registrations: Must maintain current certification in Positive Behavior Supports (PBS), CPR/First Aid, Medication Administration. Must hold a valid Ohio driver's license and possess a driving record acceptable to New Avenues to Independence, Inc.'s insurance carrier.
