Care Coordinator Job Summary

Cleveland Clinic Health System is seeking a Care Coordinator to join our team. The Care Coordinator will work collaboratively with a multidisciplinary care team to coordinate care and disease management for high-risk patients.

Responsibilities:
  • Identify patients with ongoing care coordination needs and outline the nature and duration of involvement required.
  • Conduct targeted outreach to high-risk patients to ensure timely and efficient care delivery.
  • Utilize technological tools to manage populations and conduct comprehensive clinical assessments.
  • Work collaboratively with the care team to develop goals and plan interventions to maximize patient outcomes.
  • Ensure care gaps are closed around specialty disease, chronic disease, and surgical episodes.
Requirements:
  • Current state licensure as a Registered Nurse (RN).
  • Three to five years of nursing experience.
  • Excellent communication, interpersonal, and critical thinking skills.
