About the Role

We are seeking a compassionate Hospice LPN to join our extraordinary care team at AHN Healthcare@Home. As a Hospice LPN, you will collaborate with our team to provide management and delivery of patient care plans in a team-based environment.

Key Responsibilities
  • Provide high-quality patient care in a home health setting
  • Develop and implement individualized care plans in partnership with patients and their families
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner

Requirements
  • LPN license in the state you work
  • Graduate from an approved school of practical nursing
  • Two years of LPN experience, hospice preferred
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
