About the Role:

AHN Healthcare@Home is seeking a compassionate and skilled LPN to join our Hospice Care Team. As a Home Health Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals in need.

Key Responsibilities:

  • Collaborate with our interdisciplinary team to develop and implement patient care plans
  • Provide direct patient care, including medication administration, wound care, and vital sign monitoring
  • Conduct thorough assessments of patients' physical and emotional needs
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements:

  • LPN license in the state of employment
  • Graduate from an approved school of practical nursing
  • Two years of LPN experience, with a preference for hospice care
  • Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
