Key Responsibilities:
- Collaborate with a diverse team to utilize advanced community health skills and provide technical assistance to community members.
- Mentor and support community members in accessing social and medical services, providing patient-centered care and education on health and wellness.
- Collect and maintain data while adhering to protocols and protecting patient confidentiality to address community health needs.
- Develop and implement community health programs and services to promote health equity and improve health outcomes.
- Build and maintain relationships with community partners and stakeholders to advance public health goals.
- Participate in ongoing training and professional development to enhance skills and knowledge in community health and public health.
Requirements:
- Bachelor's degree in a related field or equivalent experience.
- Basic computer knowledge and proficiency in Microsoft Office.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
- Commitment to serving vulnerable populations and promoting health equity.
- Knowledge of community health principles and practices.
- Bilingual (English/Spanish) preferred.
