Licensed Practical Nurse for Home Health Services

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CHSGa Home Health. As a LPN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities- Provide direct patient care as defined in the LPN State Nurse Practice Act.
- Participate in Home Health Services Interdisciplinary Group meetings as appropriate.
- Implement current nursing practice following a patient assessment and an approved Plan of Care initiated by the Registered Nurse (RN).
- Collaborate with the RN to review/revise the plan of care, including a discharge plan, based on the assessment of patient needs.
- Provide accurate and timely documentation of patient services to reflect the Plan of Care.
- Assess and provide patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
- Participate in coordination of Home Health agency services, appropriately reporting the identified needs for agency Home Health Services Interdisciplinary Group members.
- Use equipment and supplies effectively and efficiently.
- Participate in personal and professional growth and development. Also participate in Home Health Services Quality Management Program.
- Promote the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contribute to the work of committees, workgroups, project management, and other collaborative efforts of the System.
- Nursing degree from an accredited college or university or is a graduate of an approved LPN program.
- Current license/registration by state of Georgia as an LPN.
