Sunrise Senior Living is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for providing exceptional care and services to a consistent group of residents in our assisted living and reminiscence neighborhoods.

Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in care and service as needed.
  • Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
  • Blends Multi-Sensory Experiences: Create a variety of multi-sensory experiences for residents to enjoy.
  • Participate in ISP Development: Contribute to the development of Individualized Service Plans (ISPs) and monthly updates.
  • Responsible for Resident Care: Be responsible for the care and well-being of your designated group of residents during your shift.
  • Partner with Community Team: Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Respond to Dining Needs: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
  • Invite, Encourage, Assist: Invite, encourage, and assist residents in life skills and other life-enriching activities as indicated on their ISP and demographic profile.
Qualifications
  • Dedication to Serving Seniors: Demonstrate a passion for serving seniors with excellent customer service skills.
  • Positive Attitude and Flexibility: Possess a positive attitude and the flexibility to perform various duties in service to residents.
  • High School Diploma or Equivalent: Hold a high school diploma or equivalent, and may be required to maintain certifications in certain states.
  • Previous Experience: Previous experience working with seniors is preferred.
  • Ability to Make Choices and Decisions: Possess the ability to make choices, decisions, and act in the best interest of residents.
  • Effective Communication: Demonstrate effective written and verbal communication skills.
  • Organizational and Time Management Skills: Possess competent organizational and time management skills.
  • Good Judgment and Problem-Solving: Demonstrate good judgment, problem-solving, and decision-making skills.
