Healthcare Liaison

Bridging Care is a joint venture between two nonprofit organizations, Washington Academy of International Medical Graduates and Angels for Angels. Key Responsibilities
The successful candidate will act as a liaison with clients, ensuring they receive appropriate care and providing necessary community resources. They will work in a team with other professionals, spending most time in the community engaging directly with clients. The role involves working with medically complex patients who may face significant social barriers or behavioral health conditions.
Requirements
The ideal candidate must have a minimum of a BA/BS/BSW degree in healthcare, Behavioral Science, Social Services, or Human Services related fields. Some programs may require a degree in a behavioral health-related field. The candidate should demonstrate knowledge of behavioral and physical health issues or have the ability to learn. First Aid & CPR certification is required, or the ability to obtain within 90 days of hire. Bilingual/bicultural candidates are encouraged to apply.
The selected candidate will be able to locate and navigate community resources and services. Excellent interpersonal skills are essential, along with the ability to relate effectively and sensitively to individuals and professionals in various settings. Confidentiality maintenance is also crucial. Ability to work calmly and effectively with individuals having intensive, complex needs and multi-system involvement is required.
Salary and Benefits
This full-time position offers a salary of $30 per hour. Employees enjoy medical, dental, and vision insurance at no cost (coverage begins on the 61st day). Paid time off, mileage reimbursement, and employee referral awards are also included. A performance bonus program and employee discounts are available.
Work Setting
Care Coordinators will work in acute care, long-term care, outpatient, and rehabilitation center settings. Travel is required, with 80% willingness needed.
